# CONSENT FORM **Home Credit Mobile Application User Registration**
I am registering as a user of the Home Credit Mobile Application,
owned and operated by HC Consumer Finance Philippines, Inc. To
continue with my user registration, I confirm the following: **Data
Accuracy** 1. I warrant that all information and documents I provide
Home Credit in relation to my user registration or any subsequent
application for products and services, are true and correct. 2. I
acknowledge that it is my right and responsibility to identify and
correct wrong information submitted to Home Credit and ask Home
Credit for its immediate rectification, including personal data
received by Home Credit from third parties, and to ask Home Credit
to correct, erase or block the further processing of such
information immediately upon substantial proof that such information
is incomplete, outdated, false, unlawfully obtained or otherwise
used for unauthorized purposes. 3. Home Credit can, at its sole
discretion, automatically reject my user registration, product and
services applications, cancel its approval, and/or file a case
against me in court for cause including damages arising from false
or inaccurate information I provide. Likewise, I shall defend,
indemnify and hold free and harmless, Home Credit from any and all
actual or alleged obligations, claims, demands, causes of action,
liability, loss, cost, expense, damage and/or injury which may arise
from my breach of this clause. **Personal Data Processed** 1. I
authorize Home Credit to collect, process, use, and store
(collectively “process or processing”) my personal information and
sensitive personal information (collectively, “personal data”)
including my name, address, contact information, age, citizenship,
sex, marital status, photographs, fingerprints, other biometric data
(facial recognition and voice recognition), employment details,
income, financial data, financial profile, credit standing, loan
payment history, government issued identification numbers, and other
information required in the mobile application’s user registration
form or Home Credit product application forms. 2. I authorize Home
Credit to process Mobile device data (e.g., SIM, IMEI, or other
device identifiers, type of device, device operating system, device
settings, user account information for your mobile device or Google
PlayStore account, information about mobile network provider, device
specifications), Location data (e.g., mobile device location, time
zone setting); Phone data (e.g., contact lists, SMS metadata, types
and nature of mobile applications found on your mobile device); and
mobile app usage data (e.g., traffic volume, mobile app usage). 3. I
authorize the processing of data related to communications with Home
Credit (e.g., communication via in-app chat, email, telephone or
other channels); 4. I authorize Home Credit to process credit
information from third parties (e.g., information provided to or
from credit reference agencies or bureaus, external collections
agencies, mobile network providers). 5. I authorize Home Credit to
process the personal data of character references or any individual
whose personal data I may share to Home Credit in connection with
any product or services that I may subsequently avail after user
registration. I also warrant that I have obtained the consent of
such individuals for the processing described in this document.
**Use of Personal Data** 1. I authorize the use of these information
for the creation of my user account in the Home Credit Mobile
Application. 2. I authorize its use for data analytics, statistical
analysis and demographics, business development purposes (e.g.,
improvement of the mobile app and Home Credit’s credit scoring
algorithm, the creation of effective service delivery and payment
collection strategies, improving customer experience, customer
assistance), and the direct marketing of Home Credit’s existing and
prospective financial services as well as the products and services
of its Partners. 3. I authorize its use for fraud detection and
investigation. 4. I authorize its use for all phases of provisioning
the products and services that I may apply for including evaluation
of loan applications, its automated processing, complying with eKYC
(Know Your Customer) regulations required the Bangko Sentral ng
Pilipinas and the Anti-Money Laundering Council, credit
verification, credit scoring, and payments collection. 5. I
authorize its evaluation and use to check for my eligibility and
suitability for any potential or future cross-sell of Home Credit
products including direct marketing for other products and services
of Home Credit and its Partners and offering of existing and new
services of Home Credit. **Data Disclosure and Sharing** 1. I
authorize the disclosure to and collection of my personal data from
Third Party Entities (“Partners”) engaged by Home Credit for the
purposes stated under the heading Use of Personal Data. These
Partners shall refer to my employer (for auto-debit or other
auto-deduction mechanism) whether private or government,
telecommunication companies (e.g., Globe, PLDT, Smart, Sun
Cellular), utility companies (e.g., Meralco, Maynilad), government
agencies (e.g., SSS, GSIS, NSO, BIR), credit bureaus (e.g.,CIC,
NFIS), remittance companies (e.g., Palawan Express, Cebuana
Lhuillier, etc.), insurance providers (e.g., Sun Life Grepa
Financial Inc., Insular Life, etc.), financial service providers
(e.g., CC Mobile Financial Services Philippines, etc.), and other
service providers. 2. I authorize Home Credit to share my personal
data with Home Credit’s parent, affiliate, subsidiaries even after
my loan with Home Credit is sold or assigned by it to another
creditor for data analytics, determination of insurable interest and
amount insured, statistical analysis and demographics, and business
development purposes, and fraud detection and investigation.
**Storage and Retention** 1. I authorize Home Credit to keep a copy
of and store my personal data, relevant information, and other
documents it is able to collect for as long as is necessary for the
fulfillment of the declared, specified, and legitimate purposes
provided above, or when the processing is relevant to the said
purposes, strictly in accordance with Home Credit’s records
retention policy. After which, personal data shall be disposed or
discarded in a secure manner that would prevent further processing,
unauthorized access, or disclosure to any other party or the public.
This is without prejudice to Home Credit’s right to retain such data
for purposes of compliance to applicable laws and/or regulations,
for the establishment and/or defense of legal claims or for any
other purpose to the extent allowed under applicable laws and/or
regulations, including, but not limited to, Republic Act No. 10173
otherwise known as the Data Privacy Act of 2012, its Implementing
Rules and Regulations, and issuances of the National Privacy
Commission (“NPC”)(collectively “Data Privacy Laws”). **Client
Communication** 1. I authorize Home Credit to communicate with me
via the mobile app, phone call, SMS/text, email, mail, or any other
means of communication regarding updates to the mobile application,
updates on the status of my application to their products and
services, about the products and services of Home Credit and its
Partners, and for any other legal purpose. I understand Home Credit
may communicate with my character references and individuals
residing in my place of residence in enforcing its right to collect
on the loan and this may require sharing of my personal data. 2. I
authorize Home Credit to keep a record of all my communications with
its employees, representatives, or agents, regardless of channel
used. **Data Subject Rights** 1. I fully acknowledge that my consent
is subject to applicable laws and/or regulations, including Data
Privacy Laws, rules and regulations. As such, I am aware of the
existence of my rights as a data subject, including the right to be
informed, to access, to rectification, to erasure, to blocking, to
object, as well as the right to lodge a complaint before the NPC. 2.
While it is my right to withdraw consent to the further processing
of information about me at any time, I understand and agree that
Home Credit may lawfully continue processing such personal data for
purposes of complying with applicable laws and regulations and the
protection of its lawful rights and interests. I also recognize that
the withdrawal of such consent carries with it Home Credit’s
corresponding right to deny, suspend or terminate my registration
and my user account without need of further notice and/or demand. 3.
I am aware that I can contact Home Credit to inquire about the
processing of my personal data, for complaints about such processing
or to exercise any of my data subject rights by sending an email to
the Data Protection Officer at DPO@homecredit.ph. **Miscellaneous**
1. All of the above will remain valid even if my registration or any
application to products and services is rejected. If any of the
provisions are declared illegal, invalid, or unenforceable in any
way, it will not affect the legality, validity, or enforceability of
other provisions. Should any part of this document conflict with the
Loan Agreement, I understand that the Loan Agreement will be
